  • Services that we can arrange or pay for:
    • In-Home Care
    • Transportation
    • Escort to doctor appointments
    • Home-delivered meals
    • Respite Care
    • Supportive counseling for clients and their families
    • Other supplies and equipment

There is no cost to clients.

MSSP is a leading provider of non-sectarian case management services, helping clients to live independently in their own homes.

Our caring staff of social workers and nurses works with clients, families, and physicians to formulate the best plan of care for each person's needs.

MSSP is funded by the State of California Department of Aging and the Federal Government.

Eligibility for MSSP:

  • 65 years and over
  • Medi-Cal recipients with no medical "share of cost"
  • Functionally impaired and at risk of institutionalization
  • Can be served at home within MSSP budget limitations
  • Live within the service area
